a claim may be submitted to the C.G.M.S.A.'s insurance company.

This coverage is for House League players only,
as Travel Team players are covered by Softball Ontario.

Before filing a claim, players must contact the
C.G.M.S.A.'s board of directors, so that we can complete the form.
